Sports Registration with FinalForms

The Peru Central School Athletic Department uses FinalForms, a secure, online platform for athletic registration and forms management.

FinalForms allows parents and guardians to complete and sign all necessary athletic participation documents electronically — eliminating the need for paper packets and streamlining the entire process.

Why Use FinalForms?

  • Your information is saved from season to season and year to year.

  • Forms are pre-filled when possible, reducing repetitive data entry.

  • You can log in at any time to update your student’s information.

  • Forms must be electronically signed each year and after any updates.

How to Register

  1. Visit perucentral-ny.finalforms.com

  2. Create your account as a parent or guardian

  3. Add your student(s)

  4. Complete and sign all required forms

  5. Students will also need to log in to complete their portion

Need help? Contact the Athletic Department at 518-643-6301 or scroll to the bottom of the FinalForms page and click “Get Help.”

⚠️ Please note: All student-athletes must be fully registered and cleared in FinalForms before the first day of practice in order to participate.

Sport Level Philosophies

VARSITY

At the Varsity level, there is an increased emphasis on competition and striving to win. Participation and playing time are determined by the coaching staff based on several factors, including (but not limited to):

  • Commitment to the team
  • Athletic skill and game understanding
  • Consistent effort and work ethic
  • Positive attitude and sportsmanship
  • Academic standing and school attendance
  • Punctuality and preparation

While coaches will make every effort to develop all athletes, playing time is not guaranteed and will reflect the student-athlete’s readiness, performance, and contributions to the team.

JUNIOR VARSITY (JV)

At the JV level, competition remains important, but skill development and increased participation become more prominent. Playing time is influenced by the same key factors as the Varsity level, but there is a stronger focus on:

  • Teaching game fundamentals
  • Developing player potential
  • Providing meaningful game experience

Coaches will aim to provide opportunities for all athletes to play when possible. However, equal playing time is not guaranteed, especially in more competitive situations.

MODIFIED

The Modified level focuses on learning the sport, building teamwork, and demonstrating positive sportsmanship. Student-athletes are introduced to sport-specific skills and the structure of competitive play.

All athletes who meet academic, behavioral, and attendance expectations can typically expect to participate in each contest, though not necessarily for equal amounts of time. Playing time may be impacted by:

  • Injuries
  • Missed practices
  • Poor academic standing
  • Behavioral concerns

Coaches will make every effort to include all team members in games and practices. However, team size may be limited based on factors such as budget, facility space, and coach availability.

Cold Weather & High Heat Index Guidelines

Heat Index Procedures
Heat Index – NYSPHSAA
Cold Weather Index – NYSPHSAA

Please click the above link to view the guidelines set forth by the New York State Public High Schools Athletic Association (NYSPHAA) for High Heat and Cold Weather guidelines for athletics. The Peru Athletics Department is following these guidelines and monitoring our athletes with extreme care in these situations. Extra water and ice will be at all sites for any practice or contest in high heat conditions. Coaches will provide more breaks and have less strenuous activity as well as less duration of activity in practice. The Officials association is also building in water breaks during contests.

APP Process

The APP was revised to assess a student’s physical and emotional maturation, physical fitness, and sport skill, so that a student may be placed at a level of competition that should result in increased opportunity, a fairer competitive environment, minimized risk, and greater personal satisfaction.
